OTHER SERVICE OPTIONS:

In addition to the home office service information above, Corporation Service Company (CSC) is Pacific Life’s Registered Agent for service of process in all 50 states and their addresses are on file with the State Departments of Insurance.

Additionally, in many states the Department of Insurance will accept service of process. You should reach out to your state’s Department of Insurance for the current CSC address or Department of Insurance service of process address for your state for accurate and up-to date service of process information.

It is your responsibility to ensure that your subpoena or other service matter conforms to all applicable federal, state, and local guidelines.

REQUIRED INFORMATION:

To identify the matter in our records, it is imperative that your request has correct identifying information including:

  • Contract or policy numbers.
  • Full and complete names, addresses, dates of birth, and social security / TIN numbers of the individuals or companies whose records you seek.
  • The specific types of records that you seek.
  • The specific date ranges of those records.
  • Your production due date.

Be aware that vague, incomplete, or incorrect information may result in limited responses based on good faith interpretation of scope or may require more research or clarification, which may cause delays.

Failure to include this information may result in a delay in processing your request.

Please note:

  • Pacific Life requires a minimum of two weeks to produce records. While we try to provide records by the due date, should volume or other circumstances arise that will require more time to respond, we will contact you. 
  • HIPAA Notice: Because of state and federal privacy rules, we do not provide medical underwriting records unless there is an authorization for their release  ( jurisdiction-specific requirement). Additionally, if you are seeking medical records for a minor or for an incapacitated individual (presently or at the time of loss), we require an authorization to accompany the subpoena.

INVOICING & PAYMENT:

You will be invoiced at production. Please note as the home office records are based in California, you will be charged the fees set out in the applicable fee schedules in California Evidence Code 1563, et seq., and/or California Government Code 68093. You should refer to these codes for detail, but these fees currently include: 

  • Ten-cents per page reproduction costs.
  • Twenty-four dollars per hour clerical time.
  • Postage charges, if applicable.

DOCUMENT PRODUCTION:

Pacific Life does not provide paper copies. Pacific Life provide records through OpenText Core Share, which is a cloud-based application designed for encrypted, secure, external sharing. When responsive records are ready, you will be sent a secure access code and OpenText Core Share instructions so that you can download documents from the OpenText Core Share cloud site through your web browsers.
