How do I file a claim?

We understand that sometimes there are circumstances that can make filing a claim difficult and want you to know that we are here to support you throughout this process. You can call us at (855) 810-3301 to start a claim. To help you prepare the necessary documents, you can also download the appropriate claim package and follow the instructions inside:

  • Beneficiary Claim Package: For use by a beneficiary or group life insured who has experienced a loss. For Beneficiaries | For Employers
  • Accelerated Benefit Option Package: For use by a group life insured who has been diagnosed with a terminal illness and wants to receive a portion of their life insurance while living. For Employees | For Employers
  • Dismemberment Claim Package: For use by a group life insured with accidental death and dismemberment (AD&D) coverage who needs to file a claim for dismemberment due to an accident. For Employees | For Employers
  • Authorization for Release of Claim Information: For use by the claimant to designate an individual or entity to speak with Pacific Life on their behalf. For Claimants
  • Waiver of Premium While Totally Disabled Claim Package: For use by the claimant to have their premium waived while disabled. For Claimants
Can I update my beneficiaries?

You can make changes to beneficiaries at any time during the year. Reach out to the benefits administrator at your company to add or change beneficiaries on your policy.

Can I add or remove coverage for family members?

If your employer's plan includes coverage for family members, you likely only have the ability to make changes during annual enrollment or after a qualifying life status change, such as marriage, divorce or birth of a child. Reach out to the benefits administrator at your company to discuss eligibility and make changes.

What happens to my life insurance coverage if I retire or leave my company?

When you retire or leave the company, you may be eligible to continue coverage or convert to an individual policy. You can check your Certificate of Coverage found in your portal for details. If your coverage terminates, you will be notified of your right to convert and any other options that may be available to you.

What is Accidental Death and Dismemberment insurance?

Accidental death and dismemberment (AD&D) is a type of insurance coverage that provides a benefit in the event of a death or dismemberment caused by an accident, such as car accidents, falls, burns, and drownings.

The AD&D benefit is paid in addition to the life insurance benefit. Benefits payable due to dismemberment may cover severe accidental injuries that result in the loss of a limb (e.g., arm, leg) or the loss of a major bodily function (e.g., paralysis, sight, hearing) and may include benefits to reimburse the cost of needed modifications to one’s home and/or vehicle due to the injury, such as a wheelchair ramp or widening doors. You can review your Certificate of Coverage in your portal to see all benefits available to you.

How do I access my online portal account?

Your online Pacific Life Portal gives you the ability to manage all your benefits in one convenient location. After you enroll, you’ll receive an email invitation with a link to create your account. If you need additional support, call us at (855) 810-3301. Once your account is set up, you can sign in.
